Terms & Conditions

1. About CareBridge
These Terms govern use of care-bridge.ca and the patient coordination services provided by XHULJANO HASHO, carrying on business as CARE BRIDGE, an Ontario sole proprietorship (using the brand CareBridge) ("CareBridge", "we", "us").
- Business address: 6 Bexhill Court, Toronto, Ontario, M9A 3A8, Canada.
- Email: contact@care-bridge.ca.
- Telephone: +1 (416) 768-8186.
- Ontario Business Identification Number (BIN): 1001612858.
Please read these Terms before submitting a request. Where we ask you to accept these Terms, your acceptance relates to the services described here. A consultation request does not itself book treatment, create a payment obligation, or constitute consent to a medical procedure.
2. Our coordination role
CareBridge helps people considering treatment in Albania communicate with independent clinics, arrange appointments, understand proposed schedules, and coordinate practical support. The scope of any paid service must be set out in a separate written agreement before you commit.
CareBridge does not itself diagnose conditions, prescribe medication, perform procedures, or determine whether treatment is appropriate. Those decisions belong to the treating licensed professionals. Administrative case collection or matching by CareBridge is not a clinical assessment.
Our coordination does not create a clinician–patient relationship with CareBridge. We remain responsible for our own services and for obligations imposed on us by applicable law.
3. Eligibility and information you provide
Our standard online enquiry service is intended for people aged 18 or over who can enter a binding agreement. A parent, guardian, or other authorized representative should contact us before submitting information for someone else. Authority to consent to treatment is assessed separately by the treating provider under applicable law.
Provide accurate contact details and, when requested through an appropriate channel, relevant health information. Tell the treating clinic about medications, allergies, medical conditions, previous procedures, and changes that could affect treatment or travel. Do not submit another person's records without lawful authority.
4. Information and remote reviews
Website content is general information, not personalized medical advice. Initial reviews based on photographs, scans, or descriptions may be incomplete. Only a qualified provider can assess suitability, explain risks and alternatives, and recommend treatment. An in-person assessment may change the proposed procedure, timing, or cost.
Do not delay professional care because of website content or messages from a coordinator. This website and our contact channels are not emergency services. In an emergency, contact local emergency services immediately.
5. Independent clinics and treatment consent
Clinics are responsible for their clinical assessments, professional credentials, informed consent processes, treatment, medical records, and clinical aftercare. Before treatment, review the clinic's identity, proposed practitioner, procedure, risks, alternatives, fees, and follow-up arrangements.
Any description of a clinic as a partner, selected, or verified is not a guarantee of safety, suitability, or results. Ask us for the scope and date of our checks. Treatment abroad may involve different professional standards, complaint procedures, and legal remedies from those in Canada.
Consent to CareBridge coordination is separate from informed consent to treatment. Clinical consent must be obtained by the treating provider.
6. Prices, estimates, and payments
Website prices and comparisons are illustrative and are not a binding individualized quote. Currency, treatment components, provider assessment, taxes, and travel requirements can affect the total. Savings and results are not guaranteed.
Before any paid commitment, your written proposal must identify the supplier of each service, currency, total price or calculation method, taxes, inclusions, exclusions, payment recipient, payment schedule, and cancellation terms. Do not assume that flights, accommodation, transfers, medication, repeat visits, revisions, or treatment for complications are included.
Changes to an agreed scope or price must be explained and agreed before the additional service is provided, subject to applicable law. Nothing here overrides statutory protections relating to estimates or additional charges.
The initial case review, clinic recommendation and estimate are free. The coordination support described on our About page is included without a separate charge. This does not make treatment, transport, accommodation or excursions free. No booking or payment is authorized merely by making an enquiry; your written approval is required before a booking proceeds. Any different or additional charge must be disclosed and expressly agreed before commitment. If a referral payment or commission applies to your proposed arrangement, its existence and any effect on your charges or clinic recommendation must be disclosed before you decide.
7. Cancellations, rescheduling, and refunds
Sending an initial enquiry creates no cancellation charge. Before you pay or book, you must receive the cancellation, refund, and rescheduling terms applicable to each paid service, including any deposit, deadlines, supplier charges, and the process for requesting a refund.
Contact contact@care-bridge.ca for help identifying the responsible supplier and coordinating a request. A clinic or travel supplier's separate terms may apply to its services, subject to applicable law. These Terms do not create a blanket non-refundable deposit or remove any statutory cancellation or refund right. See also our Cancellation and Refund Policy.
If a clinic changes the proposed treatment, declines to treat you, or cannot provide a booked service, the relevant agreement and applicable law determine the available options and refund. Do not assume that travel costs will automatically be reimbursed.
8. Travel and insurance
CareBridge helps align travel dates with appointments, compare flight options, suggest hotels, and coordinate transfer arrangements. The written proposal identifies the booking provider, what is included, and what costs extra. This website does not itself issue tickets or confirm hotel reservations. Business name registration is not travel-industry licensing; no travel-registration status is represented by these Terms.
Your proposal must identify who books and supplies flights, accommodation, transfers, or excursions. Availability is subject to the relevant supplier's confirmation. You are responsible for valid travel documents and checking entry requirements that apply to you.
Discuss fitness to travel, recovery time, and timing of return flights with your treating clinician. Ask your insurer specifically whether planned overseas treatment, complications, cancellation, additional accommodation, and medical evacuation are covered. Coverage must not be assumed. Excursions must remain consistent with your clinician's recovery instructions.
9. Outcomes and aftercare
Results vary. Photographs, testimonials, and examples do not promise the same outcome for you. A procedure may require follow-up, additional treatment, or more than one visit.
CareBridge's aftercare support means helping communication and arranging follow-up requests within the agreed service scope. It is not emergency monitoring, clinical care, or a guarantee of treatment in Canada. Before travel, obtain a written aftercare plan, clinic contact details, records access arrangements, and clarity about who pays for complications or revisions.
Seek timely medical attention for urgent concerns rather than waiting for a response from CareBridge.
10. Privacy and communications
Our Privacy Policy explains how personal information is handled. Sensitive information and records will require a separate, specific consent process where applicable, including disclosure to identified clinics in Albania. Acceptance of these Terms alone is not blanket consent to share health information or receive marketing.
Use the communication channels agreed with us. Ordinary email and messaging services can carry privacy risks. Do not include detailed medical records, identification documents, or payment card details in the general enquiry form.
11. Website use and intellectual property
You may use this website for lawful personal enquiries. Do not attempt unauthorized access, interfere with its operation, submit malicious content, impersonate another person, or misuse personal information.
Website materials belong to CareBridge or their respective rights holders. You may view them for personal use, but may not reproduce or commercially exploit them without permission or another lawful basis. Third-party links are provided for convenience; third-party services have their own terms and privacy practices.
12. Service interruptions and responsibility
We take reasonable care in providing our coordination services, but cannot promise uninterrupted website availability or a particular clinical or travel outcome. If an event outside our reasonable control affects coordination, we will communicate the effect and available options. Any cancellation or refund rights continue to apply.
Independent suppliers are responsible for the services they provide. This does not exclude CareBridge's responsibility for its own acts, omissions, representations, or legal duties. Nothing in these Terms excludes liability that cannot lawfully be excluded, or requires you to waive mandatory consumer protections or remedies.
13. Complaints and applicable law
Send coordination complaints to contact@care-bridge.ca with your contact details and a description of the issue. For clinical concerns, contact the treating clinic; we can help facilitate communication. You retain the right to contact relevant regulators and pursue legal remedies.
These Terms are governed by the laws of Ontario and applicable federal laws of Canada, subject to mandatory protections that apply in your place of residence. Nothing requires exclusive proceedings in Ontario where applicable law provides otherwise, compulsory arbitration, or a waiver of lawful collective remedies.
14. Changes and separate agreements
We will post updated Terms with a new effective date. Changes apply prospectively and do not retrospectively alter an existing booking or payment agreement. Material changes to an agreed service require any notice or consent required by law.
A separately accepted service agreement governs the specific scope and commercial terms of your booking. No agreement overrides mandatory legal rights. If a provision is unenforceable, the remaining provisions continue to apply to the extent permitted by law.
